Financial Obligations

• For procedures not covered by your medical insurance, or if you are not insured, you are required to pay estimated charges upon admission. Visa, MasterCard, and personal checks are accepted.
• If you have any billing questions, please contact our billing office at (925) 280-3815
• Sequoia Surgical Pavilion bills a flat fee for each procedure performed. This fee includes use of the center and its equipment, nursing care, supplies and medications. Itemized bills are not available.
• Our fees exclude radiology, laboratory, pathology, cardiology or other ancillary services. If you have any of these services prior to, or during surgery, you will receive a bill directly from the hospital or provider of the service.
• Your surgeon, anesthesiologist, pathologist and/or other physicians bill separately for their services.
  

Will You Be Billed?
The surgery center will receive information about your insurance coverage from your physician. Our business office staff will verify your insurance benefits and notify you of any patient responsibility. You will be asked to bring your insurance card with you so we can verify the correct billing information.

Your Physician Fees
Your surgeon, anesthesiologist, pathologist, and radiologist will bill you separately based on the services they provided.

Your Insurance Coverage
As a service to our customers we will bill your insurance company. If payment is not received within 60 days of billing, the customer will be responsible for the balance.

Special Financial Needs
If you plan to pay cash, payments need to be made before or upon admission to Sequoia Surgical Pavilion. We accept cash, checks, Visa, and Mastercard. If you are unable to meet the payment requirements or have any questions regarding your financial obligation, please contact our Business Office at (925) 280-3813. Cash arrangements for anesthesia professional services must also be paid prior to, or upon admission.
