Arrival
At the arrival to the surgery suite, print your name and time on the sign in log and take a seat in the waiting area. The receptionist then will call you to start the registration process. It takes about 5-7 minutes to register. Then, sign the financial agreement which allows the facility to bill the insurance company. The anesthesiologist bills the insurance separately so you will also need to sign the anesthesia benefit assignment to acknowledge that the facility fee is solely for the facility. (This does not apply if you are an epidural or local anesthesia patient.)

If you are over 18, you will be asked if they have an Advance Directive/Medical Living Will/Health Care Proxy which will instruct the facility what to do in case the need for resuscitation arises. It will be helpful if you bring a copy with you if you have one.

If the surgery is related to a physical injury, you will be asked to fill out a form which will have the date of injury and how the injury occurred. The will expedite the claim process because the insurance companies require the injury incident if applicable. You will also be given the HIPAA Privacy Notice and be asked to sign the acknowledgement that you received the privacy notice. If you have received the notice previously another copy will not be distributed. The registration process ends with the you receiving a wrist band and taking a seat in the lobby again to wait for a pre-op nurse that will take you to get prepared for the surgical procedure.
